Tadalafil is a reversibly selective inhibitor of a specific type of cyclic monophosphate (cGMP) phosphodiesterase 5 (PDE5). When sexual arousal causes local release of nitric oxide, inhibition of PDE5 by tadalafil increases cGMP concentration in the corpus cavernosum of the penis. The result is relaxation of the smooth muscles of the arteries and blood flow to the penile tissue resulting in an erection. Tadalafil has no effect in the absence of sexual arousal.
